APPOINTMENTS:  Appointments are never overbooked at Genesis. Therefore, if you are unable to keep your appointment we ask that you call our office as soon as possible.  This will allow another patient to be seen by the doctor/therapist.  There is a charge of $75 for missed appointments or cancellations less than 24 hours in advance.  Messages may be left on our 24-hour voice mail after our business hours of 9:00 a.m. to 5:00 p.m. Monday thru Friday.  We do make appointment confirmation calls to our clients, however, we cannot guarantee we will reach you directly.  It is the responsibility of the patient to arrive on the correct day and time of their appointment. For the link to the initial appointment page click here.

PRESCRIPTIONS:   The prescription refill line is 630.653.6441 x 501. Information left on the prescription line will be addressed first  Please allow 48 hours for refills.  Calls made between Friday afternoon and Monday mornings will not be filled until late Monday afternoon.  There is a $10 refill charge, as your request requires office personnel to process.  To avoid this charge make sure you receive enough refills when you see your doctor.  The weekend on-call MD is not required to fill your prescriptions.  If they elect to do so they will only give you a few days of medicine to last until you can contact your regular MD. The $10 fee applies to controlled substance prescriptions that have to be re-written.  Please note that insurance pre-authorizations can be very time consuming and often can be done by patients.  Please do this to avoid a $25 fee.

METHODS OF PAYMENT:  We accept cash, personal checks, Visa and Master Card.  Payment is due at the time of service unless other arrangements have been made.  There is a $40 fee for all returned checks.

INSURANCE:  We accept assignment of insurance benefits only if we have a management contract with your insurance company.  You are responsible for your co-payments, deductibles and any amount not covered by your insurance.  It is the patient’s responsibility to contact their insurance to obtain any authorizations needed before being seen by their doctor/therapist.  Please be sure you provide us with any changes in your insurance so we may successfully bill your insurance company for you.
