Genesis Clinical Services Only Accepts the Following Insurance Plans: 

  • Blue Cross/Blue Shield PPO, Medicare, Medicare Advantage PPO, and Aetna PPO plans

We are not contracted with any HMO or Medicaid plans.

Medicaid requires all Providers to be in network for prescription coverage. Our prescribers are not authorized to issue prescriptions for Medicaid Patients.

Our Financial policy states all patients must have a form of payment on file. Click below to download debit/credit authorization form and email completed form to our Patient Relations team at gcsinbox@genesisclinicalservices.com. Our practice managment system is encrypted and HIPAA compliant.

For self pay patients, upon request, at the conclusion of each session, our Patient Relations Team can provide a copy of your encounter form containing all the necessary information to submit to out-of-network insurance providers for reimbursement. As plans vary, please contact insurance company to confirm the appropriate reimbursement protocol.

Once your insurance is verified and the therapeutic relationship is a match for both Client and Clinician, Our Business office will email you a new patient packet to complete on-line. Once your registration packet is sent to you through Adobe Acrobat Pro, your completed paperwork will be return to us with your e-signature. We will call you to set up your first initial appointment. We can not register or schedule an appointment prior to receiving your completed New Patient Registration Packet.
